(Music Licensing Form / Performance Notice Form)

แบบฟอร์มนี้เป็นตัวช่วยสำหรับการกรอกข้อมูลแจ้งขอลิขสิทธิ์ในการแสดง (เช่น การแสดงดนตรี การประกวดดนตรี ฯลฯ) เท่านั้น ภายหลังจากที่ท่านได้รับไฟล์แบบฟอร์มทางอีเมลของท่านแล้ว ขอให้ท่านนำฟอร์มที่กรอกข้อมูลแล้วไปติดต่อขอลิขสิทธิ์ในการแสดงจากเจ้าของลิขสิทธิ์ต่อไป

This form serves as an aid for requesting a performance license, such as for concerts, music competitions, and other events. Once you receive the completed form via email, please use it to obtain the necessary rights from the rights owner.

ข้อมูลดนตรีที่ขอลิขสิทธิ์ / Requested Music Information

ในกรณีที่ขอลิขสิทธิ์มากกว่า 1 เพลง ขอให้ท่านกรอกแบบฟอร์มใหม่อีกครั้ง (หนึ่งฟอร์ม ต่อ หนึ่งเพลง)

If you need to license more than one composition, please submit a separate form for each piece. (One form per composition.)

แจ้งขออนุญาตการแสดง / Performance Notice

ในกรณีที่ขอลิขสิทธิ์มากกว่า 1 การแสดง ขอให้ท่านกรอกแบบฟอร์มใหม่อีกครั้ง (หนึ่งฟอร์ม ต่อ หนึ่งการแสดง)
